Curis Partners and Curis Medical PLLC (“Curis”) are in-home healthcare service providers that deploy technology and a skilled clinical workforce into patients’ homes to avoid unnecessary utilization and improve health outcomes.

Our team of over 60 advanced practice clinicians (MD, PA and NP) conduct health risk assessments, provide primary care, to patients that have difficulty leaving the home, and transitional care, to patients recently discharged from the hospital. Our clinicians currently cover the 5 boroughs of NYC, Nassau and Westchester counties as well as 24 counties in the Atlanta Georgia metro area, using our proprietary EHR software to document their home visits. The Curis EHR has fully integrated scheduling and care management applications to ensure consistent monitoring and communication of the patient condition with all authorized members of the patient’s care team.

Fundamental to the assessment process is a risk scoring methodology that considers environmental, socio-economic, and clinical factors discovered during the initial home the visit to identify patients that are at imminent risk of an ED visit or hospitalization. These findings are summarized and shared with all members of the care team (PCP, Specialists, Care Managers) within 24 hours of the visit. Immediate access to this data enables the care team to collaborate and create the appropriate intervention to mitigate an exacerbation of condition.

Curis hospital clients benefit from reduced avoidable re-admissions as the result of our transitional care service. This service deploys an advanced care clinician to the patient home within 48 hours of discharge to monitor the recovery process, review the care plan and reinforce care plan adherence.
